To enroll your child for the 2013-2014 school year's tutoring program, please download the Tutorial Registration Form or stop by our office for a hard copy.  The Tutorial Registration Form must be completed and returned by a parent/guardian with original signatures only (no faxes or copies) along with all Supporting Documents (listed on the application). The child’s teacher must complete the teacher questionnaire.

Once all documents are received in our office, you will receive a call from a staff person to schedule your one-on-one interview and assessment.
